Daniel Blodgett MD permanently closed his private medical practice on December 15, 2011.

Dr. Blodgett will no longer be seeing patients, ordering tests or referrals, refilling prescriptions or giving medical advice.

Letters have been sent to all known active patients seen since January 2009 to inform them of this.

Copies of medical records can be obtained by printing a medical record release form, which is available by clicking here. Copies of charts will be available for $15 payable by check to Daniel Blodgett MD.

Inquires can be made by leaving a phone message at 559-760-7249. This phone will be checked for messages 1-2 time weekly. This web site will be maintained for one year after closing of the practice.
